About Us

The following paperwork is required on file by the 1st day of attendance:

• Child Enrollment & Health History
  
• Child Information Card
  
• please be sure to give ASC management the names of people who may pick your child up, along with the phone numbers of doctors and name of the hospital in the event of an injury.
• Signed Fee Contract (You will receive this from the A.S.C. office)
• Parent Agreement/ Tuition Express Payment Form
  
• Child Health Report
  
• Daycare Immunization Record
  
• If your child has a current IEP, we do require a copy of this to best serve each child at our facility. We also need an updated IEP every year your child is in our program.

Other required forms as needed

• Authorization to Administer Medication
  
• Sun screen Authorization Form
  
• Alternative Arrival Form/School-Age Release Form
